Services we provide:

· Occupational Therapy

Restores or improves activity of daily living: grooming, dressing, toileting, bathing, cooking, laundry management through training in use of adaptive equipments, energy conservative techniques, exercises.

· Physical Therapy

Restores or improves ability to get in and out of bed, chair to bed/toilet transfers and walking by improving strength, balance, pain; transfer and gait . Assesses and implement Fall Prevention Plan.

· Speech Therapy

Restores or improves attention, memory, problem solving skills. Provides training to improve skills to communicate and safe eating/swallowing strategies.

· Skilled Nursing Care

The nurses coordinate care, monitor medical conditions, obtain lab work, provide care and education/training for wound, catheter, ostomy management.

 · Home Health Aide

Temporary assistance provided for personal care at home , such as grooming, dressing, toileting, bathing while functional Independence obtained or alternate arrangement arranged by patient/family or through social worker consultation.

· Social Worker

The social worker will assist with long term planning, and identify available community resources to meet patient’s needs, in addition they assist in identifying social factors impeding recovery and develop coping mechanism to assist in rehabilitation. They provide assistance with living arrangements and finances as well.
